GPA is a medium sized organisation, and rapidly expanding GP Federation based in Northampton, leading the transformation of equitable healthcare across the town and beyond. The membership is a collective of 21 GP practices and covers a patient population of approximately 210,000.
Job Summary
GPA provides the Collaborative Care Team (CCT) service to support practices in the management of complex and vulnerable patients, in addition to those that require support during difficult times.
The Support Worker will work closely with the team to provide regular liaison with patients, following up on elements of their care plans and helping them to manage aspects of daily life.

Salary range £22,000 - £24,000
